Submitting a Non-Catalog Request

Follow the steps below to submit a non-catalog IT help desk request.

 

1) Go to OSUWMC Self Service Portal

 

2) Click the box in Popular Items "Click here to request an item ..."

 

3) Fill out the form as shown in the image below

 

 

Under "Description of request in detail" Please include as much detail as possible, including your computer name if applicable.

NOTE: Your cost center and fund number are required for the form to be submitted (There is no chargeback involved)
